-My Child has permission to attend the Character College Weekend Event at Community Advent Christian Church. I will not hold Community Advent Christian Church, staff, or volunteers liable for any accidents that may occur.
-In the event the applicant is injured I hereby give permission for Community Advent Christian Church to administer first aid and or select a physician to hospitalize, secure proper treatment for, and to order injections and or anesthesia and or surgery.
